Returned Goods Policy

Returned Goods
Mizuho OSI products require a Returned Goods Authorization Number (RGA #).
The RGA # must accompany the returned item(s) and be clearly marked on the outside of the shipping carton.

Mizuho OSI also requires a CERTIFICATE OF DISINFECTION FOR
RETURNED OR REPAIR SHIPMENT REQUIREMENTS document.

A minimum 10% RESTOCKING FEE is applied to ALL returned merchandise.

EXCLUSIONS INCLUDE: discontinued, disposable products, damaged or custom requests and products beyond the 30 day return policy.

 

Repairs
Mizuho OSI reserves the right to determine the cost effectiveness of repair or
replacement.

A Return Goods Authorization Number (RGA No.) must accompany
the items(s) to be repaired and be clearly marked on the outside of
the shipping carton.

To obtain a Returned Goods Authorization Number (RGA No.):
Contact an Mizuho OSI Customer Service Representative at 1-800-777-4674, M - F, 6:00a.m. - 5:00p.m. PST. Please provide necessary information such as invoice number, catalog number, quantity, date of purchase and reason for return.
