New Customer Credit Application
Are you a new customer? Complete our online credit application. Once your application is approved, you will receive a confirmation email from Dun & Bradstreet.
You count on your medical equipment every day. A service contract means you can continue to provide the best care possible, with maximum equipment uptime and worry-free functionality.
Your contract includes one annual preventative maintenance inspection per year (US only) along with service calls, travel, labor and parts/materials.
We offer full service contracts for Mizuho OSI table bases and tops.
To return an item, you’ll need to obtain a Returned Goods Authorization (RGA) number by completing a form or contacting us at (800) 777-4674 and selecting option 1, Monday through Friday, from 6am–4:30pm PT.
You’ll be asked to provide necessary information such as purchase order number, part number, quantity, date of purchase and reason for return.
The RGA number must accompany the returned item(s) and be clearly marked on the outside of the shipping carton.
Please note: We cannot accept returns on discontinued, disposable products, damaged or custom requests and products beyond the 30 day return policy. A minimum 20% restocking fee is applied to all returned merchandise.

Call us at (800) 777-4674 and select option 1. You’ll need your PO number and/or your Mizuho OSI account number.
